Outline • Background & Rationale • Objective • Materials & Methods • Results • Discussions &Conclusions
Background & Rationale • Health information exchange (HIE) is the transmission of healthcare-related data electronically across organizations within a region, community or hospital system • Interoperability is a critical element in the future success of HIE at all level • To enable interoperability between systems, the health data standards are needed
Background & Rationale • Thailand has no data standards for laboratory data at national level • Clinical chemistry lab tests are commonly ordered in healthcare facilities • Clinical chemistry lab test is the highest proportion (33%) in Thai Comptroller General Department (CGD) lab test list • The university hospitals lab test list almost coverage all lab tests in country
What is LOINC ? • Logical Observation Identifiers names and codes (LOINC) is a unique identifier for medical laboratory and clinical observations • LOINC was developed and is maintained by the RegenstriefInstitute • LOINC contains more than 72,625 terms • LOINC is broadly used in many countries • LOINC is free of charge
How does LOINC look like? • Serial code 1-5 characters and the check digit 1 character 20509-6 • The name consists of 6 axes <component>:<property>:<time>:<sample>:<scale>:<method> 2349-9 Glucose : ACnc: Pt : Urine : Ord 13457-7 Cholesterol.in LDL : MCnc: Pt : Ser/Plas: Qn: Calculated Glucose [Presence] in Urine Cholesterol in LDL [Mass/volume] in Serum or Plasma by Calculation Arbitrary concentration (+,-) concentration type When it is done Serum or plasma analyte that being measured Ordinal Mass concentration (mg/dL) Specimen type Point in time approach to do Result data type Quantitative Running number Check digit number
Objective • This study aims to study the coverage of LOINC with Thai local clinical chemistry test data
Materials & Methods • Local clinical chemistry test list (744 tests) • Ramathibodi hospital (432 tests) • Siriraj hospital (222 tests) • Chulalongkorn hospital (90 test) • LOINC database and RELMA, (was released in 29 July 2013) • Available at www.loinc.org
Materials & Methods Siriraj Ramathibodi Chulalongkorn
Materials & Methods • Preparing local test lists • Importing local terms • Verifying local terms • Mapping • Exporting mapping results • Auditing mapping results
Results • Approximately 95% could be mapped to LOINC, of which 54.84% are 1-1, 39.52% are 1-Many, and 5.64 are unmapped
Discussions & Conclusions • One to many results • LOINC has high level of granularity than local tests
Discussions & Conclusions • Unmapped results • No LOINC codes • Scope of LOINC • Drug specimen
Discussions & Conclusions • Approximately 95% mapping results suggest that LOINC is appropriate for clinical chemistry lab test data in Thai healthcare services • Mapping processes for other type of laboratory tests should be continued in order to promote interoperability in HIS
