What additional resources do you need for resuscitating a preterm newborn? • Appropriated sized ET tube(2/5 , 3 mm) • Mask with app size • Blade (size 0 .optional 00) • Surfactant • Pre-warmed incubator
How do you keep the preterm newborn warm? • Increase the room-temp 23-25 ^ C • Preheated radiant warmer • Hat • Pre-warmed transport incubator • For baby < 32 w: thermal mattresses wrap in polyethylene plastic bag
How do you assist ventilation? • If breathing spontaneously • If PPV required: use lowest PIP use a device that can produce PEEP consider administering surfactant
How much oxygen should be used? • 21-30 % for less than 35W • The oxygen saturation target range is same to term infant
What can you do to decrease the chances of neurologic injury in preterm newborn? • Handle the baby gently • Do not position the baby legs higher than the head(Trendelenburg position) • Avoid delivering excessive pressure during PPV and CPAP • Do not rapidly infuse IV fluids
