  2. TOTAL JOINT REGISTRYPhase I Patient Inclusion Criteria • Using the Knee patient triage system, patients will be prospectively pre-selected if their clinic appointment is for: • Known OA • Consideration for TKA • Previous knee surgery

All “pre-selected” Knee patients will complete tablet PC-based questionnaires • KNEE • REGISTRY • Initial Visit Data • Personal data • Knee Society Score • SF-36 • Co-morbidity data • Decisional conflict scale (before and after video0 • Knowledge verification (AFTER VIDEO0 TOTAL JOINT REGISTRYPhase I Knee patients who are felt by MD to be candidates for TKR will be identified HALF HALF STUDY GROUP CONTROL GROUP SHARED DECISION MAKING Only half of patients will be randomly selected and asked to participate in Shared Decision Making process, the remainder will not

TOTAL JOINT REGISTRYShared Decision-Making Module • Patients randomly selected for SDM module will be able to accept or decline • Those who accept: • Are enrolled into SDM clinical trial • Answer questions via Tablet PC before and after watching decision aid about: • Values influencing decision • Decisional anxiety • “Quiz” about comprehension of TKR-related issues

TOTAL JOINT REGISTRYShared Decision-Making Module • Patients randomly selected for SDM module will be able to accept or decline • Those who decline: • Are not enrolled into SDM clinical trial and do not watch decision aid (video about TKR) • Are asked to answer: • “Quiz” about comprehension of TKR-related issues • Question about why they declined invitation for SDM
