Aflac’s Personal Accident Indemnity Plan Policy benefits include: • Emergency Room benefits of $120 for the insured and spouse, $70 for each child. • Accident follow-up treatment benefit of $35 per day up to a maximum of six treatments per covered accident. • Initial Accident Hospitalization benefit of $1,000 for a 24 hour overnight stay payable once per calendar year per covered person, $2,000 if admitted directly to ICU. • Accident Hospital Confinement benefits of $250 per day: and additional benefit of $400 per day if person is confined in an intensive care unit (maximum of 15 days per covered accident for ICU) • · Specific Lump-Sum injury benefits from $35 to $12,500 based on the type of injury • Family Lodging benefit of $125 per night for a motel/hotel room to accommodate an immediate family member if accident occurs more than 100 miles from the residence of covered person. • Transportation benefit of $600 per trip to the hospital if special treatment and confinement in a hospital is required located more than 100 miles from the residence of the covered person. • Ambulance, Blood and Plasma, Physical Therapy, Prosthesis and Appliance, along with Accidental-Death and Dismemberment benefits included. • Also included is a Wellness Benefit of $60 for the insured or any one family member to undergo routine examinations of other preventative testing.

Aflac’s Maximum Difference Cancer Indemnity Plan Policy benefits include: • Initial treatment benefit of $3000 with first treatment of radiation or chemotherapy • Hospital Confinement benefits of $300 to $600 per day depending on the plan and length of stay. • Radiation and Experimental treatment benefits of $500 per week. • Chemotherepy benefits of up to $900 per week. • Nursing Services up to $150 per 24-hour day. • Surgical/Anesthesia benefits of charges up to $6,250 in or out of the hospital. • Skin Cancer Surgery benefits of $50 - $600 depending on procedure performed. • Prosthesis, surgical of $3,000; non-surgical of $250. • In-hospital and Outpatient Blood and Plasma, $150 to $250 per day. • Transportation (over 50 miles) to include personal auto and commercial travel of 50 cents per mile (commercial limited to $1,500 per round trip). • $80 per night lodging benefit • Bone Marrow Transplantation benefits of $1,000 for the donor, $10,000 for patient. • Additional benefits for Ambulance, Nursing Services Extended Care Facility, Hospice, Home Health Care • Optional First Occurrence benefits of $2500, $5000, $7500 and $10,000 includes annual $500 per year building benefit. • Optional wellness screening benefits of $50, $75, $100, or $125 per covered person per calendar year

Aflac’s Catastrophic ProgramPersonal Hospital Intensive Care Policy benefits include: • Daily Hospital Intensive Care Unit benefit of $700-$800 per day for the first 7 days. • Daily Hospital Intensive Care Unit benefit of $1,200-$1,300 per day for days 8 -15. • Benefits are payable up to 15 days per Period of Confinement. • Daily Sub-Acute Intensive Care Unit (Step-Down Units) benefits of $350 per day for the first 15 days or Days 16-30 in Hospital Intensive Care Unit Period of Confinement. • Human Organ Transplant Benefits of $25,000 per occurrence for kidney, liver, heart, heart-lung, lung or pancreas transplants with No Lifetime Maximum. • Ambulance benefit of up to $250 for ground transfer limited to two trips per confinement. • Ambulance benefit of up to $2,000 for air transfer limited to two trips per confinement.

Aflac’s Personal Disability Income Protector Plan Policy benefits include: • Benefit periods of 3, 6, 12 or 24 months. • Elimination periods of 0/7, 0/14, 7/14, 0/30, 30/30, 60/60, 90/90 or 180/180 for Accident/Sickness. • Disability income from $500 to $5,000 per month. • Definition of Total Disability: “A continuing inability to perform the material and substantial duties of your full-time job” you must also be under the care and attendance of a physician for your condition. • Pre-Existing Condition Limitation: “A sickness or an injury, for which medical advice or treatment was recommended by a physician within the 12-month period before the effective date of the coverage. Pre -existing conditions that occur more than 12 months after the effective date of coverage will be covered. Illnesses, diseases or disorders that are medically treated or diagnosed within the 30-day period after the effective date will not be covered for 12 months from the effective date of coverage.”

Aflac’s Personal Sickness Indemnity Plan Highlights of the plan include: • Three levels to choose from to accommodate all family needs. • Hospital Confinement Benefit for days 1 through 180. • Benefits for physician visits payable for sickness, accidents. • Initial Hospital Benefit can be increased with additional units up to a total benefit of $1,000 for the 1st 24 hours. • Major Diagnostic exams including: CT scans, EEG, Myelogram, Arteriogram, MRI, Thallium stress test and Angiogram. • Surgical Benefit ranging from $100 to $2,000 based on the Schedule of Operations. • Ambulance benefit: $100 for ground and $1,000 for air ambulance. Pre-Existing Condition Limitation: “A sickness for which, within the 12-month period before the effective date of the coverage, medical advice or treatment was medically recommended or received from a physician. Care or treatment caused by a preexisting condition will not be covered unless it begins more than 6 months after the effective date. Illnesses, diseases or disorders that are medically treated or diagnosed within the 30- day period after the effective date will not be covered for 6 months from the effective date of coverage.”

Aflac’s Hospital Protection Plan Policy benefits include: • Hospital Sickness confinement of $400 per day for days 1-5. • Hospital Accident confinement benefit of $500 per day for days 1-5. • Annual hospitalization confinement benefit limited to a total of five days per calendar year. • Daily Hospital Confinement Benefit of $100 per day. • Rehabilitation Unit Benefit of $100 per day, limited to 15 days per period of hospital confinement. • Invasive Diagnostic Exams Benefit pf $100 • Surgical Benefit of $50-$1,000 when a surgical operation is performed. • Outpatient Surgical Room Charge Benefit of $100-$300.

Aflac’s Voluntary Dental Plan AFLAC’s Voluntary Dental insurance is a unique schedule-based policy that can be purchased as a primary dental plan or to your supplement your existing dental program. • Benefits can be paid directly to you or a provider and premiums qualify for Section 125.
