Anthem Renewal • Received an increase from Anthem and after reviewing several benefit plan designs and the costs to employees, the Health Insurance Committee has elected a benefit change. • Considerations on the benefit plan: • Cost to make no change vs. cost to make minor changes • Deductible and Out-of-Pocket Maximum will stay the same. • Coinsurance of 20% - only applies to hospitalizations and diagnostic lab • Emergency Room copay increase to $250
Medical Benefits • * Deductible – only applies to Inpatient Hospital, Outpatient Hospital and Diagnostic Lab Testing (MRI, CAT, PET, etc.) • ** Coinsurance – once the deductible is met, then the individual pays 20% of each Inpatient Hospital, Outpatient Hospital and Diagnostic Lab Testing claim up to the Out-of-Pocket Maximum • *** Out-of-Pocket Maximum (OOP) – Includes the deductible and coinsurance
Single Example: $100 Deductible; 20% Coinsurance; $750 Out-of-Pocket Max (includes the deductible) Once the Out-of-Pocket (OOP) is met, Anthem will pay 100% of all deductible applicable services the remainder of the year.
