Medication Transfer Meds may be transferred to a different location • Day program/day hab • Another MAP registered site • Pharmacy for repackaging/relabeling • Hospital requesting supply
Medication Transfer • Current signed HCP order for the medication • Appropriate label/no hand printed changes or directions change sticker • Tamper-resistant packaging • Dated medication release document
Medication Release Document • Where the medication is being transferred to and from • Medication name and strength • Total amount of medication transferred • Dated signature of person transferring and receiving medication
Medication Transfer Medication must be transported by MAP Certified or Licensed Staff
Day Program Medication Ask pharmacy to ‘split package” medication
Day Program Medication Before Sending • Document in Medication Ordering and Receiving Log as received in residence/add countable medication to count • Complete a transfer form • If countable medication, subtract from residential program’s count book
Day Program Medication At the Day Program • Obtain dated signature of person accepting medications • If countable medication, day program staff must add to count book • Make a copy of transfer form and give to day program • Return transfer form to residential program
Day Program Communication Residential staff’s responsibility to ensure the day program has all required information for medication administration • Including current copies of individual’s HCP orders Communication is essential for safe medication administration
Central Location Disposal If medications need to be transferred for disposal to a Central Location Before Sending • Complete a transfer form • If countable medication, subtract from residential program’s count book • Transport to Central Location
Central Location Disposal At the Central Location • Obtain dated signature of person accepting medications • Make a copy of transfer form and give to Central Location • Return transfer form to residential program
Central Location Delivery If medications are delivered to a Central Location and then need to be transferred to individual residence At the Central Location • If countable medication, must be added to Central Location’s count book and subtracted when transferred to individual residence • Transfer form must be completed including dated signatures of person releasing and accepting medications • Obtain copy of transfer form for residence • Transport to residence
Central Location Delivery Upon return to the residence • Store medications securely • If countable medication, add to residential program’s count book • File transfer form at residence
