Patients working with the Practice to: Help themselves and other patients to take more responsibility for their health Provide practical support for the Practice Contribute to the continuous improvement of services Varied to suit local needs The activities of each group are determined by the needs of the community and in consultation with practice staff Based on co-operation It works by building a relationship between the practice staff and patients that breaks down barriers to communication It enables the sharing of information It can provide a springboard for public involvement in wider aspects of the NHS Awareness and sensitive It is confidential when needed It communicates widely throughout the local community A Patient Participation Group is NOT: A Forum for complaints By providing a channel for communication, a PPG can reduce the risk of complaints. Many groups agree to forward concerns to the Practice Manager or the PALS team. Implementation of a pre-determined agenda (the Governments or anyone else’s) The independence of the PPG’s is a major strength. They are informally accountable to all the patients in the practice and should therefore take a balanced view of needs. A time consuming activity for GP Practices Most PPG groups are self-organising. Information will be updated on these as and when available. Do take time out to read these boards when you are at the surgeries as they can provide useful information for you. Car Parking at Wilson Street The surgery have asked us to remind you to please not leave your car in their car park before/after your appointment. Space is limited and it has been noted that some patients are leaving their cars and going into the City Centre. We are sure you would not be happy if you were unable to park due to people being inconsiderate so we ask that you don’t take up parking spaces when you are not at the surgery. Appointments Please could you contact the surgery if you are unable to attend your appointment. This then allows them to re-book it to someone else. That someone may be you next time you are unable to obtain the appointment you request! Blood clinics Unfortunately the surgery is unable to offer blood test clinics. However you are able to have blood tests taken at the following places: Adults: London Road Community Hospital (formerly DRI) Mon – Fri 7.30 am – 4pm Walk In Centre, Entrance C, Osmaston Road Wed & Thurs 3 – 6 pm Sat & Sun 8 am – 12 noon Revive Clinic, Roe Farm Lane, Chaddesden Monday 9 am – 12 noon Thurs & Fri 9 am – 1 pm Children by appointment only at: The Walk in Centre. Care Data IMPORTANT INFORMATION ABOUT YOUR MEDICAL RECORDS Under the Health & Social Care Act 2012, NHS England will shortly be able to direct the Health and Social Care Information Centre (HSCIC) to collect and link information from all the different places where you receive care, such as your GP, hospital and community service to help provide the NHS with a full picture. This information will allow the NHS to compare the care you received in one area against the care you received in another, so they can see what has worked best. It is important that the NHS can use this information to plan and improve services for all patients. Information about you and the care you receive is shared in a secure system, on HSCIC national servers and not on GP systems. Information such as your date of birth, NHS number, gender and post code but not your name, will be used to link your records in a secure way, so your identity is protected. This information (which does not reveal your identity) can then be used by others, such as researchers and those planning health services, to make sure the best possible care is provided for everyone. The HSCIC will be the data controller for your uploaded information and will have total control over it. Once the data has been extracted, the HSCIC will be the data controller for your uploaded information and Wilson Street Surgery cannot control or protect in any way how that information is used, shared or who has access to it. You have a choice. If you are happy for your information to be used in this way you do not have to do anything.
