  2. Based on need and eligibility, Clients are assigned a Community Team Member.This person then coordinates all services for the Client. Collaborates with other agencies as needed.

  4. Advocate for clients in the following:HousingVocational assistanceSocial Support SystemsEducation

  5. The services and the need for such Services are documented in the Client’s Individual Service Plan (ISP)

  6. IMPLEMENTATION OF THE ISP GOALS AND OBJECTIVES IN THE HOME AND COMMUNITY • COLLABORATION TO THE WHOLE MULTI DISPLININARY TEAM WHICH INCLUDES: • Speech therapist • Occupational therapist • School psychologist • Psychiatrist • Therapist/social worker • Physician
