Working with the people of Camden to achieve the best health for all Service Overview CYP Community Atopy Service • Clinics Launched July 2017 • Nurse Led Service • Supporting the care needs/management for children with Asthma and Eczema within primary care settings • Established in 5 Practices Across Camden • Patients have a choice to be seen at the practice which is closest to them if the referring practices are not host to the service • Consultations and activity uploaded to Emis- accessible to practice • Referrals received from both Primary and Secondary Care

Working with the people of Camden to achieve the best health for all CYP Community Atopy Service Clinics Seen by Nurse Specialist with extensive experience of caring for atopic children in Primary, Secondary care and within the home setting 30 min Appointment slots Morning and After school appointments Offers assessment and education and support. Detailed personalised care management Plans given for Eczema and Asthma

Working with the people of Camden to achieve the best health for all CYP Community Atopy Service Links to General Paeds, A&E Dermatology, Allergy, Asthma Education sessions with Practice Nurses, GPs community Pharmacists Based within Team, home visit option available for more complex families Plans sent to teams in Camden and neighbouring Boroughs Parallel Along Baby Clinics
