Community Diabetes Team (CDT) Patients with Diabetes Have a diabetes problem not resolvable by Health Care Professional Need advice and/or considering Referral to Diabetes Centre Please discuss all potential Referrals with the Community Diabetes Team. Any inappropriate referrals will be returned. Only Refer to the Diabetes Centre for the following specialist clinics: ·Pregnancy and pre-pregnancy ·Acute Type 1 Diabetes ·Structured education for Type 1 Diabetes ·Patients suitable for and/or using continuous subcutaneous insulin infusion (CS11) ·Adolescent (non Paediatric) diabetes ·Patients with diabetes and in CKD stage 3 or lower should be discussed with CDT consultant prior to referral to specialist clinic ·Patients on dialysis Need Response Within 24hrs IMMEDIATE ADVICE For admission avoidance Call diabetes centre directly 023 9228 6260 Mon-Fri 9am—4.30pm YES NO 48hr URGENT/ROUTINE ADVICE Consultant Mobile Advice (Mon-Fri 5.30pm-7pm) Tel no: 07581 910025 Diabetes Specialist Nurse Advice Mon &Fri 8am - 4.30pm Tues-Thur 8am - 6.30pm Karen Morton 07825 119096 Deborah Fishwick 07825 118537 Jane Kendell 07887 825271 Email Consultant or DSN hp-tr.sehants-communitydiabetes@nhs.net Including: ·In subject box indicate consultant or DSN ·Your name and contact details ·Nature of problem ·Patients name, dob and NHS number ·Send via your nhs.net account We will respond TWO working days to discuss and provide appropriate advice DESMOND Referral 02392 681029 Refer to Foot MDT all patients with: ·Non-healing foot ulcer ·Acute hot swollen neuropathic foot ·Sudden change to foot shape. 02392 286260 Your consultant diabetologists are: Partha Kar, Iain Cranston, Prof Mike Cummings, Darryl Meeking, Eveleigh Nicholson

The Team • Diabetes Specialist Nurses – Deborah Fishwick, Karen Morton and Jane Kendell • Co-ordinators and lay educators - Caron Bell and Zoe Garrie • Administrators – Donna Buchanan-Bell and Jackie Cawte • 5 Consultant Diabetologists - working in the acute Trust • They also provide the same service for our neighbouring Portsmouth city community diabetes team under Solent. • Consultant team act as integrating link across organisational structures

Services we provide Patient Education DESMOND, Refresher days, Patient conference Healthcare Professional Education Diabetes essentials study days, Care home education sessions, IMIT, Nurse forums, Consultant practice visits, DUEL newsletter, HCSW conference Clinical Support Practice complex clinics, Email enquiries, Phone enquiries, ICT home visits, Virtual clinics Hypoglycaemia Pathway Working collaboratively with SCAS, Portsmouth City community Diabetes team and the Diabetes team QA Supporting Transformation Projects and New Models of Care

Our Aims • Improve health outcomes for people living with Diabetes through education, empowering them to manage this lifelong condition. • Reduce variability of care across our locality. • Reduce hospital admissions. • Increase safety for all patients especially those with frailty who are housebound. • Provide support to primary care clinicians responsible for delivering Diabetes care. • Adapting service provision to meet increasing demand.
