Why Why is it important to document? If it is not documented, it did not happen.
Tips Plan to a time to complete data entry everyday. Reread your contact notes to see if they make sense. Use a spiral or note pad to take notes. Develop a contact sheet with date, time, place ect..
Cathy Johnson, John’s Mom, called the office today at 9:30am. She spoke with Monica Garza, Center Director, and explained John has been running a fever and will not be at school today. Mrs. Johnson will take John to the doctor at 11am today. Sebrina Carroll, FSW, will follow up with Mrs. Johnson this afternoon (11-4-10) regarding the appointment.
Jennifer Molina, Jessica Ramirez’s Mom, brought in a note today from Dr. Crawford stating Jessica is Lactose Intolerant. Jessica will no longer be given milk at meal or snack time. She will be given water or juice. The Center Director and Teacher have been given the information regarding Jessica’s milk intolerance. The Head Start Nurse, Leona Davis, has been contacted so an Individual Health Plan can be developed even though Jessica has been given milk many times before and has not had a reaction.
Ashley Gonzales, Jack Simpson’s Mom, came to the FSW office today. Ashley continues to work at United and is thinking about attending classes at South Plains College this fall. I gave Mrs. Gonzales some pamphlets for South Plains College and encouraged her to come into the office and fill out her FAFSA online as soon as she completes her income tax forms in January,2011. Mrs. Gonzales will talk to her supervisor about changing her work schedule so she may attend classes. She will also contact her mother-in-law, Anita Simpson, about taking care of Jack in the evenings. I will follow up with Mrs. Gonzales on 12-15-10.
