Individuals must be 18 years of age and meet one of the following criteria: Categories of EligibilityFamily SizeRequired Documentation • Food Stamps (in last 6 months) *See definition of family - ID for applicant only ADC/TANF (current) - No proof of income - Telephone verification form or current document verifying applicant is on grant, case number, grant amount, those listed on grant, start date, etc. • Economically Disadvantaged *See definition of family - Prove all includable income for those meeting definition of family - Includable income must fall below income level guidelines according to family size • Disabled Individual - ID for applicant only - Prove income for applicant only - Verification of disability - Includable income must fall below income level guidelines for a family of one • Homeless Individual - ID for applicant only - No proof of income (estimate) - Prove homeless status
Family means two or more persons related by blood, marriage or decree of the court (including step children/step parents), who are living in a single residence, and are included in one or more of the following categories: • A husband, wife, and dependent children • A parent or guardian and dependent children • A husband and wife • Income eligibility requirements in accordance with the U.S. Department of Labor’s Lower Living Standard Income Levels (LLSIL) as of March 19, 2013: • Family Size Annual Income • 1 $11,490 2 $16,357 • 3 $22,458 • 4 $27,724 • 5 $32,715 • 6 $38,265 • 7 $43,815 • 8 $49,365 • For additional family member(s) add: $5,550.
Eligible individuals must be at least 18 years of age, low income and meet one of the following criteria: • Layoff, Termination • Permanent Closure / Substantial Layoff • Self-Employed / Out of Business • Displaced Homemaker

Adult and Dislocated Worker Programs: • Follow-up services must be made available for enrolled participants who are placed in unsubsidized employment for a minimum of twelve months following the first day of employment. A minimum of three contacts with the client must be made in the first sixty days after employment plus at least one other contact during the subsequent thirty days. • Follow-up services include, but are not limited to: • Additional career planning and counseling; • Contact with the participant’s employer, including assistance with work-related problems that may arise; • Peer support groups; • Information about additional educational opportunities; and/or • Referral to Supportive Services available in the community.
