  5. Provider • View Individual Plans • Create Monthly Reports on: • # Screened • # of Positive Screens • # of Brief Interventions, Brief Therapy and Referrals to Treatment

  6. Administrator • View Monthly Provider Reports • Create Reports on Individual Provider Data • Create Reports using Aggregate Data • No Access to Individual Screens/ Data

  7. System Login • Go To: www.collaborateandgrow.com/sbirt • Enter User ID (email address) • Enter Password
