Discuss how major services and programs for older adults that support healthy, independent and enriched living which can have an impact on the reduction of hospital readmission. 3. Connect older adults and/or their caregivers to the tools and appropriate resources. 4. Describe how the Enhanced Services Program (ESP) is a professional tool used in Georgia by the Aging and DBHDD network to locate specific services and programs. 5. Discuss the importance of how licensed Home Care Providers and Personal Care Homes self report in the ESP database if they can serve persons having a mental health diagnosis.
