Congenital: Anotia: Absent auricle Microtia: Small deformed auricle Macrotia: Abnormly large auricle Bat ear: Abnormly prominent auricle The commonest anomaly Accessory auricle: Small elevations of cartilage covered by skin in front of the auricle Pre-auricular sinus: Tiny opening leading to epithelial track in front of the Auricle (infected )

Anotia & Microtia: Plastic operation or implanted prosthesis Macrotia & bat ear: Plastic operation Pre-auricular sinus: In case of recurrent infection or sac Formation  excision of the track under operating microscope after injection of Methylene blue to delineate the track Congenital Anomalies: Treatment Prosthetic silicone ear retained by cranio-facial implants osseointegrated in the temporal bone of the skull

Collection of blood between cartilage & perichondrium Etiology: Blunt trauma (commonly in boxers) ( Boxer Ear) Clinically reddish, painfull, cystic or soft Complications : Perichondritis: due to secondary infection Cauliflower ear: due to crtlage necrosis and fibrosis Treatment : Antibiotic Aspiration: in early cases before blood clotting Incision and evacuation in late cases Pressure dressing to prevent re-collection Traumatic: Auricular Hematoma

  6. Etiology Sharp trauma as knife, bite, pulling of an earring CP: Laceration of the skin Cartlage may be involved In some cases the auricle is completely avulsed Treatment Antibiotic Immediate plastic repair Traumatic: Laceration MICROSURGICAL EAR REPLANTATION

Inflammatory disorders:Perichondritis Def : Inflammation of the auricular perichondrium Etiology: • Infected hematoma or laceration • Infected wound Symptoms: - general: FHAM • Local: The auricle swollen and painful • Signs The auricle is swollen reddish hot and tender The lobule is free ( no cartilage) Complications: - Cauliflower ear due to cartilage necrosis & fibrosis Treatment: • Antibiotic • Drainage by multiple small incisions • Debridement of necrosed cartilage

Neoplastic :Malignant Basal cell carcinoma: An ulcer with: • Raised inverted beaded edge • Necrotic floor • Indurated base • No LN enlargement Treatment: Surgical excision --------------------------------------------------- Squamous cell carcinoma - Raised everted edge - Necrotic floor • Indurated base • LN enlargement Treatment: • Surgical excision + Radical neck dissection
