Captioned Illustrations James Reason has described the safeguards put into place to prevent errors in a system as being pieces of Swiss cheese lined up in a row. There are small holes in every safeguard where something could possibly go wrong, but there are multiple safeguards. As a result, most of the time, errors and mishaps can be prevented. However, in rare cases, the holes in the safeguards line up and allow the error to slip through.
