PEEHIP Wellness Program • Fact: According to the US Department of Health and Human Services, chronic diseases are responsible for 7 out of 10 deaths among Americans every year and cause 75% of the country’s spending on healthcare. • The good news: these diseases are often preventable!

PEEHIP Wellness Program Goals of the Wellness Program: • Help members and their spouses achieve and maintain better health • Increase early detection and identification of chronic disease • Educate and encourage members towards lifestyles that lower risk factors of illness • Improve members’ overall health and energy so that they can better enjoy every area of their life
PEEHIP Wellness Program • When members maintain or improve their health, they help reduce costs for all PEEHIP members. • These savings mean PEEHIP can offer more healthcare benefits to members at no extra charge for the longest period of time. Goals of the Wellness Program:
PEEHIP Wellness Program Who is Required to Participate? • If enrolled in PEEHIP BCBS hospital/medical planand: • Active members and their covered spouses • Non-Medicare-eligible retirees • Covered non-Medicare-eligible spouses of retirees
PEEHIP Wellness Program What is Participation? • PEEHIP Wellness is a Participatory Program, which means members and eligible spouses must only participate, not meet any health standard or measurement • No results are required! • Participation is easy and will help ensure members enjoy results of living their healthiest and most capable life.
PEEHIP Wellness Program To Be Completed Before 5/31/15: • All eligible members: • Get a Wellness Screening • Complete the Health Risk Assessment (HRA) • HRA available online 1/1/2015
PEEHIP Wellness Program To Be Completed Before 5/31/15 (continued): • Only those identified as candidates: • Wellness Education Coaching • Knowledge that improves health and quality of life • Disease Management • Self examination, adherence to medication, knowing when to seek treatment, etc • Chronic diseases targeted are asthma, diabetes, coronary artery disease, congestive heart failure and COPD
PEEHIP Wellness Program There are three ways to get a Wellness Screening: • Worksite wellness screening by ADPH nurses. • Screenings are available at any Alabama Department of Public Health (ADPH) county location throughout the state. Note: If you receive an office visit referral – no copay for doctor visit if within 60 days
PEEHIP Wellness Program There are three ways to get a Wellness Screening: • Doctor’s office screening: Use our Healthcare Provider Screening Form • No copay for 1 annual preventive visit. • Note: Diagnostic services, treatments, and some lab test may cause copay charges/fees • The doctor’s office must mail or fax the fully completed form directly to ADPH. • The Healthcare Provider Screening Form is available atwww.rsa-al.gov
PEEHIP Wellness Program Wellness Screenings • Wellness screenings capture data to inform you of potential risk or illness: • Blood Pressure • Total Cholesterol (HDL and LDL) • Triglycerides • Blood Glucose • Height • Weight • Waist • Body Mass Index (BMI) • No more pre-existing conditions for any enrollee of any age • Screenings cannot keep you from being covered!
PEEHIP Wellness Program Points to Remember • Improves health by early detection and management of risk and disease • No results or improvements are required • Applies to non-Medicare members and spouses enrolled in PEEHIP’s BCBS hospital/medical plan • Screenings, HRA, Coaching, Disease Management
