Targeted Counties • Target efforts in 12 Georgia counties • Divide the 12 counties into four (4) territories • North West • North East • West Central • East Central • Quality Advisor assigned to each
Recruitment and Marketing Strategies: Physician Practices • GMCF targeted: • Physicians Practices • Community Health Centers • Health Departments • Medical School based physician groups
Recruitment and Marketing Strategies: Physician Practices • Recruitment activities included: • Face to face solicitations • Web site • Professional medical association conferences (GAFP, GSMA, GAACP) • Print advertisement in state medical journals and newsletters
Recruitment and Marketing Strategies: Beneficiaries • Recruitment activities include: • Targeted brochures for African- American and Hispanics • Located in physician offices, barber shops, nail salons, senior centers, and restaurants • Enrollment Line (888.313.WELL) • Solicitation at community events, church events, health fairs
Recruitment and Marketing Strategies: Beneficiaries • Recruitment activities include: • Physician referrals • Senior Centers / Community Health Centers • Web site • Radio, television, magazines • Posters at bus stops, grocery stores, malls
Lessons Learned • Recruitment of Providers • Face to face solicitation is the best way to recruit PPs • Attendance at provider educational meetings is a great way to speak with “hard to see providers” • Scheduling time to meet with an office manager or clinical staff persons assists with relationship building
Lessons Learned • Recruitment of Beneficiaries • Development of relationships with community partners is essential • Scheduling multiple training classes in diverse locations yields greater attendance • A reminder phone call and poster card increase attendance • Advertisement in the community increase credibility • Culturally sensitive marketing materials are important
Future Plans • Continue face to face visits to encourage providers to increase utilization on key measures • Continue to identify diverse, creative locations to hold DSME training • Continue to participate in health fairs, community and church events • Continue awareness campaign of initiative via various media outlets
Final Recruitment Targets • 181 participating practices • 80% attainment is needed • 1,839 African-American and Hispanic Medicare beneficiaries trained • 70% attainment is needed
