Ideal planning age • Because of the medical complexities of this child an ideal age would be between 14-16 years old
Transition concerns • Medical care • Equipment needs • Medical supplies • Therapeutic services • School program
Who should be involved in this process? • Parents • Primary physician • Specialty physicians • School district • Regional Center case manager • CCS therapist and nurse case manager

Challenges in finding adult care • Medical insurance not accepted • Diagnosis is too complex • Physician not accepting any more patients • Recommended physician is too far away
Success in finding adult care • Asking your child’s Primary physicians and/or specialty clinicians for recommendations • CCS staff • Peer families • Schedule an appointment with a physician that may be qualified to provide healthcare to your child
