What is an HRA? • Your employer has established a Health Reimbursement Arrangement (HRA) to help cover your healthcare expenses. • This account limits your out-of-pocket healthcare costs and is a valuable benefit. HRA Your employer contributes to your HRA. Use your HRA funds for any qualified deductible expense. Once your deductible is met, your health insurance pays per your Summary of Benefits.  Deductible Health Insurance

Accessing HRA Funds: Option 1 • Healthcare Payment Card • Use it at healthcare locations where Visa is accepted • Use it for qualified healthcare items • Automatically records purchase online • Money comes directly out of your account • No need to pay upfront and wait for reimbursement • Cannot be used at ATMs • Multiple ConnectYourCare accounts are accessible with the same card (if offered by your employer)

Frequently Asked Questions • How much will be contributed to my HRA? • The annual contribution is determined by your employer. • How will I be able to access my account funds? • You will receive a healthcare payment card from Customer Service to access your HRA funds. You can also pay for eligible expenses with any other form of payment and request reimbursement from your account. • When can I request reimbursement from my account? • You have access to the account when your plan becomes effective. • How will I know if I need to submit a receipt to ConnectYourCare for substantiation? • If a receipt is needed, you will be notified by email or letter within a week of your payment card swipe. You can also check by logging into your account and visiting the Claim Center.

Frequently Asked Questions, continued •  What if I don’t submit my receipts? • You must provide the receipts within the time requested, or the transaction will be deemed ineligible, and you will be required to refund the amount of the transaction. If you fail to submit required receipts within 60 days, your payment card will be deactivated. If you fail to reimburse the account, the amount of the ineligible expenses may be added to your W-2 or withheld from your pay. •  What information is required on the receipts? • Documentation must include the following: • Name of person who received the product or service • Date of Service • Amount • Description of product or service • Credit Card Receipts and cancelled checks are not sufficient. Examples of acceptable documentation include Explanation of Benefits (EOBs), provider bills and pharmacy receipts.

  15. Frequently Asked Questions, continued • What happens if I use the account for a non-eligible expense? • If you file a manual request for reimbursement, the request will be denied. If you used your healthcare payment card and the expense is deemed ineligible, you will be required to reimburse your account for that transaction. • When will I get my healthcare payment card? • You will receive your card in the mail about two to three weeks after the close of your enrollment period. The card comes in a white envelope. Your name and address will show through a window on the envelope. Along with the card, you will find information on how to use the card and how to gain access to your member portal. • How many healthcare payment cards will I get? • Each participant receives one card. You may order an additional card for your spouse or dependent through your online account or by calling customer service. When ordering a card in a spouse or dependent’s name, please be prepared to submit that person’s full name, date of birth and social security number.
