Mozambique Transition Goals • Goal 1 – Transition responsibility for selected ICAP-Mozambique activities to Government of Mozambique (GOM) at multiple levels for health system strengthening (HSS) and sustainability • Planning - Technical Areas (ART, PMTCT, TB, CT, Adherence, Peds, , Lab, Pharmacy) • Goal 2: Establish a new Mozambican NGO to facilitate transition by strengthening civil society

Planning Process • Step #1: Site meetings to discuss Health Facility Reports prepare by ICAP team • Step # 2: ICAP conducts district team meetings • Step #3: ICAP conducts joint provincial and district meetings Objective:Elaboration of transition workplans with full agreement by District DoH (DDS) , Provincial DoH (DPS), and ICAP

  5. Support Tool for Work Plan: Health Facility Reports (1) • Slideset used for discussion in technical meetings at site, district and provincial-levels including: • Results of comprehensive site assessment conducted by ICAP and multidisciplinary site team: space, equipment, infrastructure, patient flow, clinic organization, staffing and human resources • Routinely collected key program indicator data in graphic and tabular format (developed by M&E): • Indicators: New enrollment on ARV, Exposed Child that initiated CTX, PMTCT cascade, etc • MOC CS Bagamoio.ppt

  10. Lessons learned • Health Facility Reports and Standards of Care are useful tools for transition planning: • Promoted programmatic data-driven discussion • Buy-in for transition of CQI activities to MOH • Identified special needs on planning capacity building at districts and provincial level

  11. Next steps • Standardize graphics and tables (slide set) to include in all health facilities reports; • Capacity building in data analysis; • Develop a M&E framework at site and provincial level to monitor the transition process (outputs and outcomes indicators and targets)
