First Aid Treatment • Remain calm • The departmental first aider or first aid trained security personnel will attend casualty. • Sr. Harrop may attend casualty. • If a major injury, casualty will be transported to hospital by car or ambulance.
Reporting accidents / injuries • All injuries must be recorded in the Incident Report Book as quickly as possible on the day they occurred, or, if the incident occurred off-site, as soon as the person returns to site. • Failure to report accidents on time may result in disciplinary action being taken. • All hazards / near misses to be reported using Hazard Reports.
Completion of Incident Report Book • First Aider, supervisor or manager to complete report. • Personnel involved in incident to supply all relevant information. • Injured person / person involved in incident to sign for accuracy of details in report. • Nominated personnel to carry out an investigation.
Information to be recorded • Details of people involved e.g. name, age, address etc. • If injuries occur , specific information on the injuries is required. • What were they doing and, if appropriate, why were they doing it? • Where did the incident occur? • When did the incident occur? • How did the incident occur? • Who were present as witnesses? • Why did the incident occur?
Distribution of Report Forms • Copies of the accident report will be sent to :- • Safety, Health & Environment Manager • Health & Safety Manager • Human Resources • Medical Centre • The injured person
Accidents reportable under RIDDOR • Death • Major Injuries e.g. loss of sight, fracture, amputation. • Over 3 day injury. • Dangerous occurences e.g. flt overturning, scaffolding collapse.
