  22. Parent Information 2013-2014 FOTW “Parent” refers to a biological or adoptive parent – not a legal guardian. In case of divorce or separation, provide information about parent the student lived with most in the last 12 months. If student did not live with one parent more than the other, give information about the parent who provided the most financial support during the last 12 months. If the divorced or widowed parent has remarried, also provide information about the stepparent.
