There has been an increase in the industry for Private Investigators to be contracted to perform investigative work in claims as a SIU (Special Investigation Unit) function. Many do a good job, but there are many who do not have the experience working inside a Claims Department, either as an adjuster or SIU Investigator. The SIU Group are investigators with claims adjusting and SIU experience. Our staff are trained and skilled at reviewing a claim file, identifying issues that warrant a Referral to SIU, setting Action Plans, and conducting investigations that support and defend claims decisions. They will provide an Action Plan for investigative tasks, can give guidance and direction to the claims associate, conduct SIU Field Investigation, conducting EUOs, and participating in a Claim File Conference with the adjuster, Claims Manager, and legal Counsel. Our staff also serve as the SIU designate for your company, file the referral to the State of Florida DFS - Division of Insurance Fraud and NICB (for member companies) They will also file the Fraud plan with DIF, and conduct Fraud Awareness Training for all claims staff. We also have other investigators with years of experience and skills at conducting Surveillance.

Loss Scene Investigations, Field Investigations, and Claimant Recorded Interviews $ 85 per hour • A summary update will be provided 3 days or less and the final report, with photos and any statements within 7 days. • Our Investigators are trained and experienced in diverse situations requiring investigation, particularly relating to insurance claims, civil litigation, and criminal matters. 8. $ 600 Flat Rate, for 8 hours on site. • $ 85 each additional hour requested. • A verbal and email report of the results will follow the operation within 24 hours. • The Investigative report will be provided within 3 days of the completion of the assignment via email. The hard copy report with any video documentation on CD will be provided within seven days. Surveillance

9. $ 600 Flat Rate * This Flat Rate applies to EUO assignments that do not involve field investigation activity. Rate is for file review and preparation, EUO notification, and conducting EUO, and summary report. $ 50 charge for live streaming, If you choose to use that added Service. Examination under oath

10. Ever wanted to have someone in person at a EUO, but travel and time constraints prevented them from being present? The SIU Group uses a company to provide live streaming of the EUO. You can sign in from your office and monitor the EUO as well as IM the Investigator with your input. Several people involved in the claim can sign in and view the EUO. This is a valuable tool in conducting EUOs. It gives you control at your convenience.

11. Clinic Inspections: Flat Rate $ 500 or $ 85.00 per hour Clinic Inspections include a site visit by the Investigator with photos of the facility exterior and interior (interior when permitted). The Investigator will compile a list of all equipment used and connected to billing procedures. The Investigator will also report on all licensed and non-licensed personnel at the facility and a licensure check of staff. Inspection will also include a database search and court records check of all identified owners or corporate officers. CLINIC INSPECTIONS

12. Background Checks: $ 50 Flat Rate The Background Investigation will include: Basic B/I or locate charge is $50 for data base search for current address, alternate addresses, court records search, and any other data base runs. Multiple person checks of 5+ at rate of $35 each. A summary report is provided with this service. BACKGROUND CHECKS

13. Hourly Rate for Investigative Work $ 85 per hour Travel time and mileage will be charges for work outside a primary city of coverage. The hourly rate plus (IRS Mileage rate of $ 0.56 per mile).
