How: Consultation published and extensively circulated Version made available on-line + Easy Read Six public consultation events held around the county Contributions and Responses: 30 RAT members & 100+ people at Stakeholder event 46 individuals submitted written responses 5 organisations/groups submitted written responses 200+ people took part in the public “events” The consultation

What the consultation told us • Widespread support for the general priorities • A greater priority for improved community support services for people living at home (including intermediate care) • More detail on how progress will be monitored and reported • More emphasis on how joint commissioning will work in practice • More focus on the timing of improvements within the pathway • The need for a personalised service • More clarity about the financial profile

Simplified Care Pathway Primary Prevention Early Diagnosis & Support Living Well with Dementia End of life care Ongoing wellbeing Personal Support Services Carers Strategy Care in General Hospitals Care between hospital and home Housing Support and Telecare Living Well with Dementia in care homes Palliative stage Raising Public Awareness and Understanding Early Diagnosis and intervention Easy access to care, support and advice Peer support from local people with experience of Dementia Improved End of Life care Patient and carer planning Across the Life Course Good quality information for patients and carers + An informed and effective workforce for people with dementia + A Joint local commissioning strategy for dementia

What we have done so far • Professional & public awareness improvements • ‘Memory treatment' services operating in each locality • Primary care link worker for older people pilot • Dementia Advisor pilot across all Norfolk localities • Draft Carers strategy consistent with joint Norfolk dementia strategy • Each Acute General Hospital has a nominated lead clinician for dementia

And….. • ‘This is me’ booklets are being used • Mental health liaison nurses in acute settings • Protected meal times on all wards • Workshops for Care Home Managers across all sectors  • Reduction in the use of anti-psychotic drugs in Care Homes • NWMH NHS FT established in-reach support to Care Homes • SWIFT workforce development plan project for MH including dementia

What you told us you wanted!! • Increase Memory Clinic Capacity • Improve /Increase Educational Support & Materials • Improve Capacity in Cognitive Stimulation Therapy • Improve Prescribing • Raise GP Awareness • Improve services for people with dementia in Acute Hospitals • Support initiatives that will improve the care people receive in Care Homes • Review Community & Day Care including where appropriate respite
