2. What are WellFirst Discount Plans? • The WellFirst Discount Plans offer one of the most extensive consumer-driven savings programs in the nation to individuals, employers and their employees. By aggregating the purchasing power of hundreds of thousands of consumers nationwide, we enable our clients to take advantage of deeply discounted rates typically reserved only for insurance companies and Fortune 500 employers

The Benefits of Discount Plans • Easy to use with no health restrictions • Use immediately and begin saving as soon as you receive your card • No paperwork, you pay the discounted amount at the time of your visit • Use the card as many times as needed • One membership for the whole family • Avoid unnecessary trips to the doctor This is not insurance nor is it intended to replace insurance. You do not need insurance to purchase these plans. Plans only available in CT, NY, NJ, MA, NH, OH, IL, and TX

4. Flexible Enrollment Options • Two ways to enroll: • Voluntary • Employee paid through payroll deduction (after-tax only) • Monthly list bill - List bill can be via mail or email • List Bill has a $50 monthly minimum • Direct Bill is available • Employer paid

WellFirst Discount Plans • Total Wellness ($11.95* a month) • Chiropractic Care - Members save 30% to 50% at over 3,000 participating Chiropractors nationwide • Dental Care - This dental network provides members with discounts of 15% to 50% on dental services at over 92,500 available dental practice locations nationwide • Vision Care - Members save on eyeglasses, contacts, eye exams and surgical procedures by utilizing a national network of providers • Consult A Doctor - Offers 24/7 access to its proprietary nationwide network of U.S. licensed physicians for telephone and secure e-mail medical consultations • Pharmacy - The neighborhood pharmacy program assures members the lowest price on prescription drugs, saving 10% to 85% on most prescriptions. • Hearing Aids - Enjoy access to a variety of hearing aid discounts of 15% to 58% in retail locations nationwide and discounts of 40% to 60% through the mail order program • VIP Health and Wellness - Save on retail vitamins and nutritional supplements as well as diabetic supplies and skin care products through preferred vendors *A $3.95 one time, non-refundable application fee not included

WellFirst Discount Plans • Personal Wellness ($12.95* a month) • LifeLock - If you become a victim of identity theft while a member of LifeLock due to a failure in their service, they will spend up to $1 million to cover the cost for lawyers, investigators, case manager, but not any direct losses as a result of the theft • Legal Care Direct - Plan attorneys will either charge $125.00 per hour, or 40% off their usual and customary hourly rate, whichever is greater  • Financial Helpline - Financial Helpline members have free access to experienced, accredited financial counselors • Pet Care - With the Pet Care Savings Program there is a 25% discount at network veterinarians on most procedures, plus savings of 10% to 30% on pet products and services at participating merchants • Tax Help Line - Members of the Tax Help Line will have access to unlimited tax advice, prior year tax return review and free tax return preparation for forms 1040 EZ, 1040A and standard 1040 • Cash Back Saver - Enjoy great deals, huge savings, and Cash Back when you shop at your favorite online stores *A $3.95 one time, non-refundable application fee not included

9. Implementation • Implementation • Prior to enrollment, the following information will need to be provided to WellFirst: • Application for Group Number • Employer Agreement for List Billing (Recommend first of the month effective dates as monthly fees are not prorated) • WellFirst will provide: • Group number • Group specific marketing materials • For ease of enrollment, we have developed a computer program that allows us to enroll your employees electronically • Signed employee payroll deduction forms • Employee contributions are made on an after tax basis • Since each discount card requires a one time, non-refundable application fee of $3.95* the employer will need to set up a pre-funded account to cover the one time fees. • *This fee must be paid by the employer in order to activate the card.

10. Administration • Administration • The pre-funded account needed to pay the one time application fee will be debited at each enrollment, so the employer will need to continue funding once the account is depleted. • New Benefits will mail or e-mail a list bill on a monthly basis • Terminations need to be communicated to WellFirst via email prior to the last business day of the month in order for the group not to be billed for that month (billed in arrears)
