The number of Follow-up Allied Health Services for People of Aboriginal and Torres Strait Islander Descent (MBS Item 81300-81360) provided to patients in the Division. The Department will provide MBS data. Divisions will be asked to comment on changes over time and discuss successes and challenges.

Strategies used to: identify and assist individuals, families, or communities who would benefit from access to health checks and/or other health services; and - ensure that Indigenous Australians attend appointments and access referred and/or prescribed treatment.

Number of Indigenous Australians assisted with: • attendance at initial appointments with primary health care services. • attendance at: • follow-up GP, specialist, care-coordination, allied health appointments, visits to the practice nurse. • c) filling and collecting prescriptions for medication.
