HFMEA • HFMEA stand for Healthcare Failure Mode and Effects Analysis. • It is an analysis of potential failure modes within a system for classification by severity or determination of the effect of failures on the system. It is widely used in manufacturing industries in various phases of the product life cycle and is now increasingly finding use in the service industry. Failure causes are any errors or defects in process, design, or item, especially those that affect the customer, and can be potential or actual
Implementation of HFMEA Step 1- Select a Process – Select the process you want to examine. Define the scope (Be specific and include a clear definition of the process or product to be studied). • Step 2 - Assemble the Team - Assemble the Team – Multidisciplinary team with Subject Matter Expert(s) plus advisor Step 3 - Graphically Describe the Process Step 4 - Conduct the Analysis Step 5 - Identify Actions and Outcome Measures
Implementation of HFMEA • Step 3 A & B. Consecutively number each process step. 1 2 3 4 Hot bath Wear your cloth Prepare your coffee or tea Drive your car
Step 3C & D. If necessary, list sub-process steps and consecutively number. 1 2 3 4 Hot bath Wear your cloth Prepare your coffee or tea Drive your car • Sub-processes: • Prepare your • towel. • C. Check the water • temperature. • Sub-processes: • A- Iron your cloth • Check the color • if it fit well • Prepare your • makeup (women only) • that suit the cloth. • D- wear your cloth • Sub-processes: • Add coffee or • tea to the coffee • maker. • B. Prepare your cup • C. Add sugar • D. drink your coffee or • Tea • Sub-processes: • A. Check your keys • B. Check the gas level • C. Start your engine • D. Drive your car. Scope
RPN • The Risk priority Number (RPN) is calculated for each of the causes. RPN is calculated by • (Severity) X (Occurrence) X (Detection) • Highest RPN 1000 lowest 1
5 Whys Analysis • The 5 Whys is a technique used in the Analyze phase of the Six Sigma DMAIC methodology. It's a great Six Sigma tool that doesn't involve data segmentation, hypothesis testing, regression or other advanced statistical tools, and in many cases can be completed without a data collection plan.
5 Whys Example • Problem Statement: You are on your way work in the morning and your car didn’t work.1. Why did your car stop? - Because it ran out of gas.2. Why did it run out of gas? - Because I didn't buy any gas on my way to home.3. Why didn't you buy any gas this morning? - Because I didn't have any money.4. Why didn't you have any money? - Because I spent it all last night purchasing new computer.5. Why did you spent it all your money new computer? - Because I upgrade the options and cost me more than I expected.
Finding & Recommendation • Improvement in profit • Costs reduction • Wastages reduction • Cycle time improvement • Improved customer satisfaction • Improved services and product • Understanding of Customer requirement
