Group Members • MichaDhondt– Nutritionist from Belgium • Rianne Nicolai – Nurse from Holland • Sandra Wojdat– Nurse from Denmark • Sue Hibbard – Learning Manager from Australia
Establish a WIKI • http://mrss4.wikispaces.com/ • This link is available in IHCP Group1 folder, is open to public viewing and limited to member contributions. • Web based tool where everything is in one place on-line.
Key facts • Second leading cause of accidental or unintentional injury deaths worldwide. • Globally, an estimated 424 000 individuals die from falls • Adults older than 65 suffer the greatest number of fatal falls. • 37.3 million falls that are severe enough to require medical attention, occur each year. • http://www.who.int/mediacentre/factsheets/fs344/en/index.html
Facts about Falls. • Consequences of injuries are far more serious. • Increased mortality • Decreased mobility • Reduced social contact • Increased anxiety (Evron et.al 2009) • Leads to significant health, human and economic costs. (WHO 2007)
“The Question” • How can we as professionals in an interdisciplinary team enhance the physical condition and rehabilitation of elderly clients who are effected by falls?
Project structure • Introduction – for whom the problem is important • -why the problem is important • -why the chosen focus of the problem is of interest professionally and interdisciplinary? • -definitions of falls, rehabilitation • A fall is an event which results in a person coming to rest inadvertently on the ground or floor or other lower level” (WHO 2011)
Rehabilitation • "Rehabilitation is a person-centered, active and creative process that involves adaptation to changes in life circumstances. It is a shared activity between the ... person, people close to them, and multi-professional teams who recognise the contribution of all concerned," (RCN, 2000). Hawkey,B & William,J. 2007,Role of the rehabilitation nurse. Royal College of Nursing http://www.rcn.org.uk/__data/assets/pdf_file/0017/111752/003178.pdf
Project Structure (cont) Methods – - roleplay - mindmap -case study - literature review Theories - Healthy Ageing Model – motivational interviewing (MI) - Stages of Change - Theory of Reasoned Action/ Perceived Control
Why use a ‘Case Study’. • Limits the extent of the question by constructing and framing the problem. • Forming the case requires viewing the story from several perspectives eg. nurses/nutritionists. • Cases take on a ‘life of their own’ when discussed with the Interdisciplinary Group. • Discussion leads to reflective practice. • “Findings – Case writing uniquely facilitates reflection-in-action and the building of communities of practice.”(Meyer, Heinz-Dieter; Shannon, Brenda. Journal of Educational Administration, 2010, Vol. 48 Issue 1, p89-101, 13p, 3 Charts; DOI: 10.1108/09578231011015430)
Project Structure (cont) • Case Presentation - Mr Kajewskihas been admitted to hospital after fallingin his home and injuring his hip. He is 79 years old and his wife of 58 years died 6 weeks ago. He has 2 adult children with families who live nearby. In recent weeks he has not felt like eating very much and is not skilled in cooking. He weighs 70 k and is 1.86m with a BMI 20.3. Six months ago his BMI was 23.2 (weight 80k). After being operated on and cared for in the ward for six days, Mr Kajewskiwill be transferred to the rehabilitation unit.
Project structure (cont) • Analysis and Discussion • examine the problem by combining and analysing and discussing the data with the chosen theory. • Problems will develop during research of literature and discussions with Mr Kajewski and the interdisciplinary team. • Healthy Ageing Model. Stages of Change, Theory of Reasoned Actioned.
Project structure (cont) • Conclusion – answer the problem question • References • Appendices • Individual evaluation of process
Evaluation • Repeat the question and evaluate perspectives concerning the project :- what should we improve in the future with respect to interdisciplinary cooperation and the problem area, areas that should be looked more into in the futureusefulness for the professionsnational/global perspectivesstrong and weak points concerning the project methods or theory.
