  2. การรักษาในผู้ใหญ่และเยาวชน (เด็กอายุ>12ปีขึ้นไป)เริ่มART • CD4 < 200 • มีอาการ(เชื้อราในปาก,PPE rash prolong fever , chronic diarrhea > 14 วัน , , , wt loss >10 %ใน 3 เดือน) • AIDS defining illness

  3. กลุ่มยา • NRTIs AZT D4T 3TC ABC TDF ddI • NNRTI EFV NVP • PIs LPV/rIDV RTV NFV ATV SQV-sgc

  4. Follow up • Clinical • Adherence • Adverse effect ของยา • OI • Reponsive + resistance? CD4 ทุก 6 mo. viral load หลังเริ่มยา 6 เดือน ต่อมาทุก12 mo.

  5. Adverse effect • Short term( เกิดใน 6 เดือนแรก) GI AZT NFV rash NVP EFV neuropsychiatric disturbance EFV • Long term lipodystrophy NRTIs PIs metobolic DM PIs NRTIs osteopenia PIs • Short term Long term mitochondrial toxicity NRTIs hepatotoxicity NNRTIs PIs Renal IDV TDF
