PCI in patients with ACS and diabetes • Patients with ACS plus diabetes are at higher risk for recurrent events but derive greater benefit from aggressive therapy • Mainstays of acute-phase therapy in diabetic ACS: • Triple antiplatelet therapy: Aspirin, clopidogrel, GP IIb/IIIa inhibition • Heparin or LMWH • Early invasive assessment and, if appropriate, stent-based PCI • Despite sharp declines in restenosis rates with drug-eluting stents, patients with ACS plus diabetes remain at high risk for repeat revascularization Roffi M, Topol EJ.
