CHIC Focus Areas Patient Centered Medical Home (PCMH) Demonstration Develop a Pediatric Practice Improvement Partnership in Idaho (NIPN visit 11/11) Promote the use of health information technology in children’s healthcare (EMR, HIDE, IRIS, Medical Home Portal) Annual Learning Collaborative Sessions (Asthma 9/11) • CHILDREN’S HEALTHCARE • IMPROVEMENT COLLABORATION • www.CHICProject.dhw.idaho.gov

Idaho Medical Home Demonstration Selected pediatric practices (3-4) in Idaho will receive: Learning Collaborative Sessions Embedded Medical Home Coordinator (shared with another practice) Help in selecting, training, and supporting a Family Partner Consultation and assistance from QI Practice Coach Support in building infrastructure and systems to improve efficiency, support care coordination, and measure clinical and service quality Opportunities to share experiences, resources and lessons learned with other practices • CHILDREN’S HEALTHCARE • IMPROVEMENT COLLABORATION • www.CHICProject.dhw.idaho.gov
Participation Requirements • Pediatric or Pediatric Subspecialty Clinic based Practice • Electronic Medical Record with registry function • 3 year commitment (32 months with embedded care coordinator) • Physician Champion • Ability to travel a core practice team 1-2 times per year during the demonstration • CHILDREN’S HEALTHCARE • IMPROVEMENT COLLABORATION • www.CHICProject.dhw.idaho.gov
