  1. 感染性生物材料輸出入申請流程 (簡單四步驟) • 1. 帳號申請: 登入疾病管制局”感染性生物材料簽審通關系統”，以實驗室主持人名義申請帳號。 • https://bioaudit.cdc.gov.tw/cdc_doh/login.aspx • (經疾病管制分局人員核准使用者帳號，審核時間約需一天) 2.帳號核准後開始線上填寫申請表格 • “海關報單納稅義務人統一編號”欄位，請填進階統編:23348952 • 輸入國別請填寫”US”(由美國輸入台灣) • 若購買的數量大於一，請選擇”多次”輸入 • 若有多項生物材料，請逐一”增列項次“輸入貨品欄位，切勿填在同一欄 • 3.列印申請表格並簽名 • 4.以機關(構)公函檢送相關文件函送疾病管制局(總局)-10050 台北市中正區林森南路6號 • 申請表格 • 申請輸入之生物材料若為安全等級2(含)以上(見附件一)，需另檢附貴單位”生物安全委員會同意書”(附件二) (Ex. Lentiviralpartical是安全等級2的範疇，需另外檢附生物安全委員會同意書。)
