Services Each site’s services are unique to the site and driven by community need but all major sites include Primary Care as defined by a FQHC Adult Care - Pediatric Care - Behavioral Health- Dental Health-WIC- School Based Health-Adult Day Care- Drug Court Behavioral Health-Family Planning Services Public Health-Podiatry-Cardiology-Nephrology
The first site in Luling with Public Health at the focus. Later added Norco and Kenner and the Day Care Center. • School Based Health Center at Albert Cammon School, new Grant to develop another at Paradis. • 29th Judicial Drug Court Services.
Summer 2009, opens , 1 Pediatrician, WIC Unit, 3600 sq ft • Summer 2011, move , 1Pediatrician, 1 Nurse Practitioner, WIC Unit, .2 Psychiatrist, 1 Counselor, Public Health Site (Family Planning, Immunization) 8000 sq ft • Summer 2012 , 1 Pediatrician, 1 Nurse Practitioner, 1.5 Family Medicine MD, WIC Unit, .25 Psychiatrist, 2 Counselors, Public Health Site, 1 Counselor for City of Slidell Drug Court Services, 8000 sq ft • 2013, Covington Site Change in Scope filed-to open April-May. 2010 Acquire the Site from FMOL and operate as is- 4 Physicians, 3 Nurse Practitioners, Lab and Radiology Services • 2012 Cross over the Center to an FQHC, staff still the same, some services dropped due to new Hospital opening, GNOCHC program accepted, still in trailers. • 2013 Jan-Feb preparations to move into new location in MOB of Hospital. Plan for 2 Physicians, 3 Nurse Practitioners, Behavioral Health Providers, WIC and St Bernard Parish Sanitarians at site. Unique Patients Served at Site • 2011- 12,847 in Primary Care, 1,183/ GNOCHC • 2012- 12,204 in Primary Care, 1,891/ GNOCHC
2012 1st FQHC Opens in Plaquemines Parish at Belle Chasse. Currently housed in space formerly occupied by Coroner of the Parish who now works as a Family Practice Physician for the CHC. A Pediatrician is also practicing at the site. Collaboration efforts are underway to solidify the clinic as well as grow its services into the near future.
