  1. InspireMD Perforation and Aneurism Off label use case reports of the MGuard stent

  2. Case #1 - perforation • 86 year old female • Mixed mitral valve disease, paroxysmal AF and coronary disease • Severe LAD disease • Recurrent admissions to hospital with pulmonary edema • PCI to LAD to improve symptoms and prevent hospital admissions • Troponin 0.09. Baseline creatinine ~ 150 • Coronary perforation covered with MGuard stent • Serial ECHO examinations showed a small pericardial effusion with no change over time

  3. Baseline angiography Pre-perforation post-perforation

  4. MGuard deployment Pre-deployment, the MGuard is positioned in the perforation area (blood jet is showed) M-Guard is deployed and leakage stopped

  5. Final Angiography Post-dilatation Final result showing a patent artery with no sign of perforation

  6. Case #2 - perforation • 61 yr old male • Ex-smoker • Laryngectomy for carcinoma • Limiting angina with positive ETT • Declined CABG

  7. Baseline angiography Pre-perforation First sign of perforation located near a bend in the artery

  8. MGuard deployment Pre-deployment, the MGuard is positioned in the perforation area M-Guard is deployed

  9. Final Angiography Post-deployment, showing the MGuard position Final result showing no sign of perforation

  10. Case #3 – coronary aneurysm • 68-year old male • Previous angioplasty on obtuse marginal branch • Admitted for angina and a positive myocardial perfusion imaging test (reversible perfusion defects on inferior wall) • Angiography showed a patent stent on obtuse marginal artery and a large coronary aneurysm of the right coronary artery (6.7 mm in diameter and 15.2 in length) • Post deployment hospital stay was uneventful and the patient was discharge the day after the procedure with dual antiplatelettherapy • At 1 month he was totally asymptomatic

  11. Baseline angiography Pre-treatment 3.5X24 MGUARDstent was implanted at 16 bar

  12. 1 month post procedure Angiographic coronary CT

  13. Case #4 - brain aneurysm Pre procedure MGuard in position Final result
