3. Road Closures • Prior to closing a road you must notify your contractor coordinator. • This includes both full or partial road closures • The Communication Center (757-380-4031) has to be notified prior to any road closure. • They notify the fire department and security (O15) • Base One (757-380-2461) must also be notified. • They notify emergency personnel such as: • Water Tender, Emergency Electrician, and Spill Response Teams • Attempt to always leave a lane or enough space open for emergency vehicles when performing a road closure.

4. Vehicle Parking on Crane Travel Path • Do not park vehicles on the yellow travel path of cranes • Material should not be delivered or stored on the travel path of the crane. • Always be aware of the crane envelop.

Contractor L&H Operations - General Overview Approved Safety Plan L&H Section in Approved Safety Plan Crane Operations X36 Notification Certificate of Compliance -3rd Party Certification Operator Qualification – 29CFR1926.1427(a) AD Director Rigger Qualification 29CFR1926– “Qualified" means one who, by possession of a recognized degree, certificate, or professional standing, or who by extensive knowledge, training, and experience, has successfully demonstrated his ability to solve or resolve problems relating to the subject matter, the work, or the project Applies to both crane L&H operation and non-crane L&H operations

6. Contractor L&H Operations Lift Plans • Lift plans are required at NNS for all L&H operations, not just crane L&H operations. • Where can you get help with lift plans? Rigging Gear • 29CFR1926.251 - Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. • "Inspections." Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a “competent person” designated by the employer. • “Competent person" means one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. • For Steel Erection - A qualified rigger (a rigger who is also a qualified person) shall inspect the rigging prior to each shift in accordance with § 1926.251.

7. Contractor L&H Operations • Hoisting Personnel • 29CFR1926.1431 • Only permitted when conventional methods for personnel access is more hazardous or  not possible. • Total load on the lifting equipment must be 50% or less of the rated WLL of the lifting equipment crane in the radius and configuration to be used except for proof testing. • Lifting equipment must have controlled load lowering.  No free fall permitted. • Lifting equipment pre-lift operational check of required devices • Personnel Platform requirements • Rigging gear requirements • Proof Testing • Trail Lift and inspections • “Competent” person responsibilities • Working Under Suspended Loads • NNS does not allow personnel under suspended loads except where an hazard evaluation has determined that the work cannot be performed in another manner or the alternate methods poses a greater hazard to personnel.  This non-allowance includes the exceptions noted in 29CFR1926.735. • The evaluation, lift plan and a detailed rigging sketch for the lift must be reviewed and approved by both the responsible Field Engineer or Contract Coordinator and X36 L&H Engineering is required.  All information shall be at the lift site during the lift.

8. Contractor L&H Operations L&H Accidents • Your responsibilities when a possible L&H accident occurs: • Make the site safe (emergency response, etc). Do not disturb the site any more than necessary to make the site safe. • Contact the Service Desk at 688-9888 and provide the requested information. • Wait for the Accident Prevention Team (APT) to arrive on site. • Follow the APT instructions.

9. Storm Drain Protection • Straw bales cannot be used on construction sites at NNSB. • A bundle of straw bales caught fire recently at DD12. • The bundle of straw bales was being used to protect storm drains/sumps during construction operations. • Alternate methods must be used: • Silt bags • Filter cloth

10. Excavation Permits • Recent Excavation Permit Issues: • No signature for RadCon or O27. • No E-mail confirmation. • E-mail must be placed with excavation permit. • No Project Coordinator signature approving work to commence. • No General Contractor signature. • No worker signatures on permits.

11. Foreign Exclusion Devices (FMEDs) • FMEDs are available at the O46 conex box located east of the Leyte Boiler platform (North of B4613). • Your coordinator must send a charge number to Steven Thomas Steven.M.Thomas@amsec.hii-nns.comor 757-534-4093 • All piping shall come fromthe supplier with FMEDs. • If supplier did not provideFMEDs then the contractormust place FMEDs on the material before it comes into the yard.

12. Fuel Gas Hoses in Confined Spaces • Fuel gas and oxygen supply lines and torches. • Fuel and oxygen gas cylinders and distribution manifold systems can not be located in enclosed or confined spaces. Maintain them in open areas. • All fuel gas and oxygen hose lines must be disconnected from the source at the end of each shift. • All fuel gas and oxygen hose lines shall be rolled back to the supply manifold or to open air prior to disconnecting the torch. • Fuel gas and oxygen hose lines shall not be used unless they have been drop tested to ensure the integrity of the hoses. • Drop test needs to be preformed for at least sixty (60) seconds.

13. Fuel Gas Hoses in Confined Spaces • Fuel gas and oxygen supply lines and torches. • No unattended fuel gas and oxygen hose lines or torches are to be located in confined spaces. • H&SM, Vol II, Ch. 5 • A qualified Fire Watch, who has been fully instructed in emergency procedures, shall be stationed at the entrance to the confined space. • The fire watch shall have a hose clamp loosely placed around the torch lines to be able to rapidly shut off the gases in an emergency. • The fire watch shall not leave his/her station while the operator or his/her equipment remain inside the confined space. • The Fire Watch shall be positioned to be able to observe or converse with torch operator.

14. Ladder Safety • Inspect ladders prior to use. • Ensure rungs, and steps are in good condition and free of grease, oil, and dirt. • Ensure locking devices and non-skid safety feet are in place. • Place ladders on a firm footing. Do not use or place a ladder on a slippery surface. • Maintain a 3-point contact. When ascending or descending a ladder, face the ladder and maintain a 3-point contact. • Two hands and one foot or one hand and two feet. • Ladders shall be secured by both side rails at the top to prevent displacement. • Ladders shall extend 3 feet above the landing surface. • Ladders shall be placed with a one to four ladder slope. • The bottom of the ladder should be one foot away from the wall for every four feet that the ladder rises. When a ladder slope of one to four cannot be attained for ladders up to 12 feet in length, the bottom shall be secured. • Do not stretch beyond the side rails. Doing so could cause the ladder to tip and fall. • Only one person is allowed on the ladder at one time. • Be observant for electrical wiring and third rails in the vicinity of where the ladder will be used. • A step ladder is a work platform: • Never stand or sit on the top of a stepladder. Stepladders have a label that states, “do not stand above this step.”

15. Sound Horn Before Passing Equipment • Recent near misses involving vehicles passing JLGs and forklifts with out warning. • EH&S Manual Vol II, CH 13 • Operators of vehicles must sound their horn and pass on the left around JLGs, forklifts, scissor lifts, etc. • O43 has begun installing decals on mobile equipment for drivers to sound their horn before passing.

16. Hot Work & High Visibility Vests • Recently there have been observations of personnel performing hot work while wearing high visibility vests. • These high visibility vests are made of 100% polyester. • Polyester when heated can melt and can adhere to the skin. • Use non-synthetic fabrics when performing hot work. • Use proper PPE when performing hot work: • Based on your company’s PPE hazard assessment.

17. Genie Boom Lift Recall • Z-135/70 • The secondary boom angle sensor calibration was performed improperly. • An improperly calibrated machine can lead to machine instability resulting in a machine tip over. • It is required the immediate removal of all Genie Z-135/70 Boom lifts from service. • Machines may only be returned to service after the inspection listed below is performed.

18. O15 Security Background Investigations • Huntington Ingalls Industries has recently awarded a contract for performing background investigations (BI) on our employees and visitors to General Information Services, Inc. (GIS), trading as “backgroundchecks.com”. 19. O15 Security Badge Process The 9327 Process Is The Same: • Visiting company (Contractors) conducts background investigation (BI) on their employee who will be badged to work at NNS • Visiting company completes 9327 and delivers to NNS Sponsor in the same manner they have done in the past • NNS Sponsor or NNS VMS user enters visit into VMS in the same manner they have done in the past • NNS Sponsor or VMS user maintains 9327 (for the required period of time) as they have done in the past • Visiting company maintain file of complete background investigation on employees as they have done in the past

20. O15 Security Badge Process What’s New: • Visiting company prints “certificate of BI completion” and turns in with 9327 (Rev 11) to NNS Sponsor   • Visiting company employee brings copy of certificate of BI completion to Access Center when he/she is ready for NNS Contractors Badge. This is a one page document NOTE: Visiting Company Employee Does Not Bring 9327 To Access Center NOTE: Visiting Company Employee Does Not Bring Background Investigation To Access Center In order for the Access Center to process a visit and issue a badge: The visiting employee must have a current visit in VMS. The visiting employee must produce picture ID. The visiting employee must produce proof of citizenship. The visiting employee must produce the a copy of the certification of BI completion

23. Security Badge Process • All BIs must be performed by the Huntington Ingalls Industries corporate award supplier, which is currently General Information Services, Inc., trading as backgroundchecks.com. • A new BI is required for all unescorted badge visitors every three (3) years, and following any break in service in excess of 90 days, to re-verify criminal and DMV checks

24. NNS Standard Time-and-Material TicketOverview • The NNS Standard Time-and-Material Ticket shall be used to document all T&M work performed on NNS contracts. • A fully completed T&M ticket shall be submitted daily to the NNS Field Engineer daily. • A blank Microsoft Excel 2007 formatted version will be provided to the Contractor by the coordinators. • All fields must be completed prior to submittal to NNS. • All subcontractor back-up must be attached to the T&M ticket.
