This initiative aims to prevent Ventilator Acquired Pneumonia by tracking key process measures like bed elevation, ulcer prophylaxis, and monitoring clinical outcomes such as VAP rates. A dashboard with real-time alerts helps in compliance monitoring.

Process and Outcome Measures Ventilator Management Redesign Effort
Process and Outcome MeasuresExample • AIM: To prevent Ventilator Acquired Pneumonia. (Outcome Measure). • Measure: Number of patients with a diagnosis of pneumonia who are on ventilators (and were not admitted with pneumonia). • Changes: There are 5 different changes that will be implemented and therefore 5 process measures: • head of bed elevated, stress ulcer prophylaxis, DVT prophylaxis, oral care, and sedation management.
These are the five processes that will be monitored (process measures) since they are predictors of ventilator acquired pneumonia.
Ventilator Management Dashboard • Process Indicators – Real-time system with alerts (red, green, yellow flags) indicating compliance with the Ventilator Management Bundle.
Process measures should have well defined steps. The flowchart on the next page provides that information.
