Required Reports • All programs receiving Violence Against Women (VW) funds have to complete a VW Annual Report and Performance Reports (Bi-Annual).
Violence Against Women (VW) Annual Report • VW Annual Report covers: January 1, 2011 - December 31, 2011 • Due Date: February 15, 2012
VW Annual ReportGeneral Info Sec. AI - Pg 1&2: • Date of Report: Date you complete it • Year: 2011 • “Subgrantee” refers to your agency/program. • Subgrant #s – The TWO contract numbers for VW for the calendar year (VW-11-_ _ and VW-12-_ _) • Type of funded organization – check all that apply • Point of contact - • Tribal Populations – Answer Yes or No (VW funds specifically for this purpose)
VW Annual ReportGeneral Information Pg 2: • Percentage of VW program grant funds are directed to: • Sexual Assault (SA) • Domestic Violence (DV) – including dating violence • Stalking • Percentages add up to 100%
VW Annual ReportA2. Staff Information Pg 3 • Report the number of Full Time Equivalents (FTEs) with VW funds • You can use increments • Report in decimals and not percentages (i.e. .25 rather than 25%)

VW Annual ReportE1. CJ System - LE • The first question deals with AGENCY-wide statistics • The remaining questions report ONLY VW statistics – NOT agency statistics.
Example of Agency vs. VW • Example: • All calls that came in for the entire agency (VW funded and all other officers): 10 Domestic; 6 Sexual Assault; 2 Stalking. • Count all those numbers on first question. • From that point on, it’s important to know how many out of the 10 DV, 6 SA and 2 Stalking were responded to using VW funds. Those are the statistics we are collecting.

Not just the person who signed. We pay tribute to those doing the work….. • Preparer’s Telephone/Email: ….however, we will then call you with questions.

Performance Report • Example: • Officer All Crime responds to 10 domestic calls and takes 4 reports, and Officer VW responds to 20 domestic calls and takes 10 reports. • Under Table 1: 30 calls for assistance under DV and 14 reports under DV. • Under Table 2: 20 calls for assistance under DV and 10 reports under DV.
