Why should an SBHC care about medical homes? • The Medical Home Model is being adopted by both public and private payors as a means of providing cost-effective quality care • Accountable Care Organizations are forming to provide comprehensive care to communities through a variety of medical homes • SBHCs have focused on cost effective quality and PREVENTIVE health services for a long time • If an SBHC is not participating within their community’s health care delivery system, they will be limited to a first-aide/urgent care model of care

YES • Participate in state meetings to assist in the development of state-specific medical home standards or requirements • Conduct a Gap Assessment of any proposed medical home standards to the SBHC models in your state • Analyze any steps necessary for SBHCs to participate in the state medical home model. • Provide technical assistance to SBHCs regarding key actions they must take to participate in the medical home model.
