Coal Mine Fatal Accident 2012 (No.1) OnSaturday, March 03, 2012, a 32-year old section foreman with 11 years of experience was fatally injured while installing a metal canopy over the operators compartment of a shuttle car. He was positioned in the operators compartment of the car underneath the suspended canopy, when the supports broke and allowed the canopy to fall and strike him resulting in fatal injuries. The hoist used to position the canopy was anchored to a roof bolt bearing plate. This anchor point was not able to support the weight of the canopy and over a period of time broke. There were no secondary cribbing material or means of support placed under the suspended canopy. OVERVIEW

Coal Mine Fatal Accident 2012 (No.1) • Mine personnel shall be properly trained in all job tasks to be performed. • Suspended loads shall be securely supported by a substantial means: capable of bearing the load. • Now work shall be conducted under suspended loads unless a secondary means of support is provided. • Repairs or maintenance shall not be performed on machinery until the power is off and the machinery is blocked against motion.
