Summer Requirements • Districts shall offer a minimum of 50 hours per subject of intense and focused instruction in ELA and math. • Districts must have a written attendance policy that explains how many of the required 50 hours constitute attendance. • Students must attend 80% of the required hours equal to 40 hours.

Paperwork Guidelines • Only original paper work will be accepted. • NO whiteout, or scratch outs • Paperwork cannot be faxed. • Tutoring timelines must coincide with official school day. • Employee numbers must be included and match employee names. • Officialteacher and student names must be used. • All employee signatures must be in BLUEink only.

  19. Paperwork Details LEAP Tutoring Pay Form • Employee Sign In • Must be Handwritten in BLUE ink • Form must be totaled. • Form must be signed by the LEAP Lead on line below last entry • Cross out all lines below the signature.

  20. Paperwork Guidelines • Students must initial sign-in calendar in the correct date. Pencils are allowed for students only. • Correct dates must be used. • Forms must returned in correct order to expedite processing. • All forms must be submitted by deadline.
