Community Telehealth History • Community Home Health began our telehealth program in 2007 with Heart Failure patients • We have utilized the same vendor for telemonitoring with devices since beginning of program • Currently, we have over 100 telemonitoring units in patients homes • We added an interactive voice response system (IVR) in 2011 with non-homecare patients

IVR Results First Pilot Results: • Patients were engaged in self care and self reporting of weights and symptoms • IVR system worked with minimal system errors • Satisfaction scores showed 90% of patients rated satisfaction of 8 or higher on a scale of 1-10. • Able to track specific NP and RN interventions via the IVR system. • Example: one time med orders was most common NP intervention
