Meaningful Use Quick Hits This guide should be viewed as a very simple, quick “how to” guide for helping you pass each measure. Wave Two is part of the North Texas Regional Extension Center, partnering with the EHR team to consult with providers who are participating in the EHR Incentive program established by the HITECH Act to achieve MU.

Meaningful Use Quick Hits • Core Measure #3: Problem List • Discrete documentation that the patient has either “No Known Problems” or an ICD-9 or IMO Problem IT diagnosis
Meaningful Use Quick Hits • Core Measure #4: ePrescribe • e-Prescribe all eligible medications from encounter • Avoid “calling in” or handwriting medications • Monitor staff usage
Meaningful Use Quick Hits • Core Measure #5: Medication List • Use “No Current Meds” button in History OR • Enter medications discretely in either History or A&P, No Free Text
Meaningful Use Quick Hits • Core Measure #6: Medication Allergy List • Use “NKA” and/or “NKDA” button in the Allergy section OR • Enter all allergies discretely (environmental and medication)
Meaningful Use Quick Hits • Core Measure #7: Demographics • DOB, Gender, Race, Ethnicity, & Preferred Language • Review the Demographics tab • All fields except for Preferred Language should populate from Flowcast, build adding language into staff workflow!
Meaningful Use Quick Hits • Core Measure #8: Recording Vitals • Height, Weight, and Blood Pressure must be entered for each patient seen at least once during the reporting period • “Patient Reported” is an option
Meaningful Use Quick Hits • Core Measure #9: Smoking Status • You must use “Tobacco Use” and a detail from the “Tobacco Use Details” folder • If tobacco use has been documented in any other fashion in the past, edit the existing item
Meaningful Use Quick Hits • Core Measure #10: Clinical Quality Measures • Total of 44 CQM’s, Providers must report on 6 total measures • 3 required core measures (substituting 3 alternate measures if excluded) + 3 additional measures • Not a measure of performance in stage 1, just have to be able to report your data • Wave Two will be working with each clinic to develop these reports • Core Measures (FYI)- • Adult Weight Screening and Follow up • Hypertension: Blood Pressure Management • Preventative Care and Screening Measure Pair • Tobacco Use Assessment • Tobacco Cessation Intervention

Meaningful Use Quick Hits • Core Measure #13: Clinical Summaries • Add “Clinical Summary (v9.2)” to Print/Fax Pack & use on each encounter- • Draft button (in A&P) or at bottom of sign off screen • This document pulls sections of the A&P, Vitals, & RFV screens • Set options to always print this document

Meaningful Use Quick Hits • Menu Set Measures Overview • 10 measures in all, you must meet only 5 of them. • Measure #1 (Formulary checks), #3 (Patient Lists), and #10 (Syndromic Surveillance) are by attestation • This leaves us with our choice of 2 additional measures to meet
Meaningful Use Quick Hits • Menu Set Measure #4: Patient Reminders • Create report based on any criterion (keep in mind that you need patients under 5 years old or over 65 years old) • Work that report through the “Report Results” area • You can call patients on this list, send letters, etc. As you are reaching out to the patients on this report, mark a status
Meaningful Use Quick Hits • Menu Set Measure #6: Patient Specific Education • Adding patient education documents to the A&P is not enough! You must output the documents
Meaningful Use Quick Hits • Menu Set Measure #7: Medication Reconciliation • Measures all patients with “Transition into Care” in the Reason For Visit section and the “Medications Reconciled” button clicked in the History- Medications section
Meaningful Use Quick Hits • Menu Set Measure #8: Summary Care Record • For any transitions out of care, check the assume management box on the referral pop up, or sign off as “Transition of Care” AND output an H&P document
