pharmacodynamics • Racemic form of ondansetron- a selective blocking agent of serotonin 5ht3 antagonist
pharmacokinetics • Excreted renally – patients with mild to moderate renal impairment have a 2-fold increase in retention time – mean half life increases to 11.6 hrs versus 5.7 hours in normal • No reduction in dose or dosing is called for in these patients
Indications/Contraindications • INDICATED FOR : • Intractable vomiting and/or vertigo • Prevention of postoperative vomiting / nausea • CONTRAINDICATION • Known hypersensitivity to the drug
Protocol • Routine paramedic care • Consider IV access and fluid resuscitation for dehydration and/or shock • For patients with patent airway and retractable vomiting: • Administer Zofran 4 mg ODT • Do not push through the foil backing – peel away • IMMEDIATELY place the Zofran ODT on top of the tongue where it will disolve in seconds • Giving with liquid is not necessary • No repeat is indicated
BE ALERT FOR • Signs and symptoms of dystonic reaction • Administer 50-100 mg of benadryl IV or deep IM
