Project Description / Scope of Work • City of Phoenix wastewater collection system includes over 94,000 manholes and structures • Manhole and structure construction materials include brick, cast-in-place concrete, and precast concrete • Manholes may have no lining, PVC-lining or epoxy lining • Manhole rehabilitation primarily consists of surface restoration and application of corrosion resistant coatings to walls and inverts • Rehabilitation may include structural inserts or replacement of manholes or structures.
Project Description / Scope of Work • Provide oversight of the project construction activities to ensure protection against defects and deficiencies in the work of contractors and their subcontractors. • Inspect the site through various stages of construction to inspect the quality of the executed work and to verify the work is in accordance with construction documents and specifications. • Perform field inspections (visual, probe, and testing) where required for completed repairs and reject work not conforming to project documents and specifications • Collect and provide asset management documentation on all adjustments, replacements, and modifications to existing facilities. • Track, compile, and categorize all project related documents, including inspection reports, progress photos, important project correspondence, meeting minutes, RFIs, Submittals, etc., on a CD, following completion of the project, and deliver to the City Project Manager. • Provide public outreach
Statement of Qualifications Criteria • General Information – 10 points • Construction Administration and Inspection Experience of Prime Firm – 25 points • Experience of Key Personnel & Subconsultants - 30 points • Project Understanding & Approach – 30 points • Overall Evaluation of the Firm – 5 points
Statement of Qualifications Criteria • Each page side with criteria information will be counted. • Pages that have photos, charts, and graphs will be counted toward maximum number of pages. • Cover letter, front and back covers, Table of Contents and tab dividers will NOT be counted toward the maximum page limit, UNLESS they include information that may be considered by the selection panel.

Grounds for Disqualification • Receipt of Bid after the specified cut-off date and time • Deposit of Bid in the wrong location • Violating the “Contact with City Employees” policy
