  5. Step 2/Group I • If they are in Group I—check the box(es) on the GREEN Cover Sheet. 1 (one) white Scholarship Application will be turned in. Even if you check 20 scholarships to apply for in Group I, only 1 copy of your Scholarship Application and essays is necessary.

  6. Step 2/Group II • If they are in Group II—check the box(es) on the GREEN Cover Sheet. 1 (one) white Scholarship Application will be turned in for every box you check. If you check 20 boxes, you will turn in 20 copies of your Scholarship Application and essays

  7. Step 2/Group III • If they are in Group III—check the box(es) on the GREEN Cover Sheet. Scholarships in this section will require a Supplemental Form in addition to the Scholarship Application. In order to apply for Group III Scholarships you must walk into Student Services between Rooms G02 and G03 and retrieve the Supplemental Form. This is mandatory. A Scholarship Application will be turned in for every box you check. If you check 20 boxes, you will turn in 20 copies of your Scholarship Application, the essays AND the corresponding Supplemental Form

Step 3 • Ask 3 MHS Teachers to fill out a PINK rating sheet for you. • We ask that YOU fill out the top portion before handing it to the teachers. • Please do not ask K-8 teachers or counselors • Only 1 of the 3 can be one of you coaches while at MHS • The teacher/coach will turn in the PINK rating sheet directly to Ms. Anderson

  9. Step 4 • Complete the WHITE Scholarship Application. • You will have the document emailed to you. • There are 3 essay questions on page 2. You must answer 2. Save the responses in a Word document. You will need to print them when printing the Application. • Do not forget to sign and date page 3. • You will print the application and make copies. When doing so, don’t forget to print the essays as well to make a complete Scholarship Application.

  10. Step 5 • Turn in on or before March 5th at 4:00 pm to Ms. Anderson, Mrs. Meeker or Ms. Ford in Student Services. • We will count the number of applications in comparison to the scholarships applied for before you leave • The deadline is final.
