Firstly: Field work • Field Officers • Home and office visits, phone consultations, InterRai contact assessments • Home visits assessing both the Pwd and Carer • Identified needs and risks • Referrals on to other services e.g. NASC, respite coordinator, other community groups • Provide updates to GP’s with recommendation i.e. referrals to Elder Health, continence support etc. • Carer referrals – have needs in their own right often not met • Support into day programs to encourage socialisation also respite use • Taxi chits assessments
Education/advice about dementia • Understanding the disease and behaviours • Legal issues EPOA, Driving • Financial supports available • Planning for the future respite and residential care options • Revelant brochures and information • Wanda Trak– tracking system for recidivist wanderers living within the community.
On going support • Ongoing home visits or phone contact • Advocacy/Liaison • Supporting both the Pwd and the carer • Support groups • Monthly for the carers, • Socialisation – MKC Club and walking group • Pwd introduced to daycare services e.g. chatham Club and/or other providers appropriate placements • Networkworking • Other services and supports for all those affected by dementia • NASC, MHSE, GP’s, Practice Nurses Provider agencies, NGO’s etc…
Secondly: MKC Club • Day club specifically for Members (people with dementia) • Client joins as a member of a club • Stand alone facility with a home like environment • Members are encourage to help each other and staff within the club • Staff experienced and trained to work with those affected by dementia
MKC Club • Program developed around the members • The blokes shed this also houses a pool table • Transport pick up and deliver home for most members • A hot meal for lunch • An environment where the member feels empowered to function without fear mistakes or embarrassment.
MKC Club • RESPITE • Regular respite for the carer • Feedback positive wellbeing of both Member and carer improves, the member looks forward to club days and is often more motivated than has been previous to club involvement.
