Two main types of human errors: Judgement -Thought it was safe -Thought I had enough time Memory - Did not remember procedure - Remembered the wrong procdure Perception -Did not hear alarm -Did not see signal -Did not perceive danger Action -Opened wrong valve -Pressed wrong button Deliberate human errors: Human errors by intent; for example shortcuts, silent deviations etc. Not focus in this CAKE module Unaware human errors: Human errors because of distracted choices: It is human to make mistakes However, it is also human to learn from own and others’ mistakes in order to achieve an injury-free workday Focus in this CAKE module is on errors because of personnel not being TOTALLY AWARE.
