Figure 1 Background information, instruction to subjects As a neonatologist or a fellow in neonatology, you are asked to perform an urgent consult for a 35-year-old woman, G1P0 at 24+6 weeks of gestation in preterm labor of unknown etiology. Tocolytic medications have failed. Preterm premature rupture of membranes occurred 12 hours ago and the mother’s cervical dilatation is 7-8 cm. Delivery is expected within the next few hours. Three days ago she was admitted to the hospital for observation and has received a full course of bethamethasone. Because you have another urgent consult to do, you have only 30 minutes to accomplish this task. Additional information: The fetus is a product of in vitro fertilization therefore gestational age is exactly 24+6 weeks. The fetus is a boy with an estimated birth weight of 550 grams. The fetal monitor shows a normal fetal heart rate with good variability. You are working at a tertiary care hospital. Maternal medical history is unremarkable. In terms of social history, the mother is a schoolteacher and father is working abroad (en route back to the country at the time of your consult).
