Features of the project • CBCHS – 3 locations – Clarinda, Parkdale , Chelsea • Divisions – Bayside GP Network / Monash – 9 practices • Major partners to be engaged – Southern Health, RDNS, Local Govt • Other agencies and services which express interest in participating
Broader aspects • Linkages with the KBPCP HP activities and early intervention approaches • BAEW – promoting physical activity and healthy eating for children in KB • Active for Life – physical activity, falls prevention and older persons • Community Connectedness – responding to social isolation within the catchment • Future Focus for Youth – identifying young people at risk • Talking Realities – peer education program for single young mothers
